However, this goal can often feel daunting due to the mounting challenges faced by independent pharmacists today.

With declining reimbursement rates and escalating burnout among staff, the viability of these pharmacies is at risk. Additionally, the aggressive tactics employed by pharmacy benefit managers (PBMs) are intensifying the financial strain on these small businesses.

When these pharmacies close their doors, the impact is deeply felt in the surrounding communities, often creating “pharmacy deserts.” In these areas, residents must travel long distances to access essential medications, making it even harder for rural Floridians to maintain their health. A recent Associated Press report highlights several such areas, especially in Northwest Florida and the Northern Panhandle.

The question then arises: who is responsible for addressing this issue—lawmakers or the pharmacies themselves? The answer is both. While legislative action at the state and federal levels can alleviate some of the pressure, a major source of support for these pharmacies comes not from government, but from Pharmacy Services Administrative Organizations (PSAOs).

PSAOs are crucial allies for independent pharmacies, offering business support in many ways—from tracking performance and helping with compliance to negotiating with PBMs and other insurers. While they may not be directly involved in patient-facing services, PSAOs ensure that pharmacists can focus their energy on the health of their communities by handling time-consuming administrative tasks.

Perhaps most importantly, PSAOs are indispensable in the ongoing fight for the survival of independent pharmacies. Though they do not influence medication pricing or pharmacy earnings, PSAOs work tirelessly on behalf of pharmacies, advocating for their interests in a healthcare system that grows more competitive each day.

It’s time we recognize the critical role of these partnerships so that all Floridians, no matter where they live, can access the healthcare they need and deserve.
