Aggressive Cholesterol Lowering May Help Prevent First Heart Attacks in Patients with Diabetes

A major new analysis from the VESALIUS-CV trial, presented at the American College of Cardiology 2026 Scientific Session, suggests that intensive cholesterol-lowering therapy may play a much earlier role in preventing heart attacks—especially in people with diabetes who have no prior history of cardiovascular disease.

For decades, powerful cholesterol-lowering drugs have largely been used after a person has already experienced a heart attack or stroke. However, this new research challenges that “reactive” approach and supports starting aggressive prevention much earlier in high-risk patients.

A Shift from Treatment to Prevention

The study focused on a subgroup of 3,655 patients with diabetes who had no known history of heart disease. Participants were already receiving standard statin therapy, but researchers added evolocumab (Repatha), a PCSK9 inhibitor that significantly lowers LDL cholesterol.

The results were striking.

Patients who received evolocumab experienced a 31% reduction in major adverse cardiovascular events (MACE) compared to placebo. These events included heart attack, stroke, and cardiovascular-related death.

Researchers also observed a consistent benefit across different outcome measures, including procedures such as coronary revascularization.

Understanding How Evolocumab Works

Evolocumab is part of a class of drugs known as PCSK9 inhibitors. These medications help the liver remove more low-density lipoprotein (LDL), often referred to as “bad cholesterol,” from the bloodstream.

While statins remain the first-line treatment for lowering cholesterol, they may not always be sufficient for individuals at higher risk—particularly those with diabetes.

In this study, patients already on optimized statin therapy saw their LDL levels drop dramatically:

  • From a median of 111 mg/dL
  • To approximately 44 mg/dL after 96 weeks

This level of LDL reduction is typically reserved for people who have already experienced serious cardiovascular events, making its use in primary prevention especially notable.

Stronger Protection Over Time

One of the most important findings was that the benefit of treatment increased with time. While early reductions in cardiovascular risk were already evident after one year, the difference between the treatment and placebo groups continued to widen, reaching up to 40% greater protection after the first year.

Researchers believe this suggests that long-term LDL lowering may provide accumulating cardiovascular protection rather than just short-term benefit.

Rethinking Current Guidelines

Current cardiovascular guidelines generally recommend moderate-intensity statins for patients with diabetes who do not yet have diagnosed heart disease. More aggressive therapies are usually reserved for those whose cholesterol remains elevated or who have already had a cardiac event.

However, this study suggests that waiting for disease to develop may mean missing a critical window for prevention.

Experts involved in the research argue that earlier, intensive intervention could help prevent the first heart attack rather than simply reducing the risk of a second one.

Safety and Tolerability

The study also provided reassuring safety data. Serious side effects leading to discontinuation were similar between groups:

  • 4.1% in the evolocumab group
  • 4.3% in the placebo group

Overall, the treatment was well tolerated, even when used alongside high-intensity statin therapy.

Limitations to Consider

While the findings are promising, researchers noted some limitations:

  • The study population was predominantly White (93%)
  • It focused specifically on patients with diabetes
  • Not all participants underwent detailed imaging to detect hidden arterial plaque

Despite these factors, researchers argue that the study still reflects real-world conditions, where advanced imaging is not routinely performed for primary prevention.

The implications of this research are significant. It suggests that for high-risk patients—particularly those with diabetes—lower LDL targets and earlier use of intensive cholesterol-lowering therapies may offer stronger protection than previously believed.
